UnitedHealthcare At Home helps make at-home care simple — and it all starts with an in-home visit.

Step 1: Your first visit
Your first visit will include up to a full hour of 1-on-1 time with a member of our licensed medical staff. You’ll get a physical, health screenings and plenty of time to talk about your health questions.

Step 2: Regular visits
After that, you’ll receive regular in-home visits with UnitedHealthcare At Home either at least once a year or more frequently with ongoing medical care in your home. It all depends on your needs, and we work around your schedule.

Step 3: Ongoing support
You’ll work with your care navigator to help coordinate your ongoing care needs. They can also connect you with information on resources that may be available to you, like transportation services.

Plans are insured through UnitedHealthcare Insurance Company or one of its affiliated companies, a Medicare Advantage organization with a Medicare contract. Enrollment in the plan depends on the plan's contract renewal with Medicare.
*A member must complete a UnitedHealthcare At Home visit through HouseCalls within the calendar year to be eligible for the $50 reward. Please allow one week after the completion of your UnitedHealthcare At Home visit for the reward to be loaded to your UCard. If you have questions about your UCard, visit the UCard hub on your plan website or call the number on your UCard. Use your reward on approved items at a participating retailer near you, including nationwide chains and neighborhood stores.
